Association between sleep quality and cardiometabolic risk in adults: Evidence from recent observational studies

Abstract

This integrative review analyzes the association between sleep quality and cardiometabolic risk in adults, a topic that is relevant to disease prevention and clinical risk stratification. The study aims to synthesize recent observational evidence on the relationship between sleep quality, considering subjective and objective measures as well as aspects of duration and regularity, and different cardiometabolic outcomes. To this end, it conducts a search in the PubMed, Scopus, and BVS/LILACS databases from 2020 to 2025, selecting studies based on PECO criteria and a systematized methodological quality assessment, resulting in the inclusion of 11 observational studies. The findings indicate a consistent association between poorer sleep quality and a higher risk of metabolic syndrome, type 2 diabetes mellitus, arterial hypertension, and composite cardiovascular risk. In addition, this relationship is identified through both subjective instruments, such as the Pittsburgh Sleep Quality Index, and objective measures, such as polysomnography and actigraphy, although heterogeneity is observed across instruments and cutoff points. It concludes that the evidence supports the incorporation of sleep assessment into clinical practice and highlights the need for standardized longitudinal studies with more complete reporting of effect measures.
