Abstract

Obesity and overweight are major global public health concerns, associated with metabolic alterations such as insulin resistance, dyslipidemia, and systemic inflammation. In this context, dietary strategies such as intermittent fasting (IF) and continuous caloric restriction (CCR) have been widely investigated. This study aimed to compare the effects of these interventions on metabolic outcomes in adults with overweight or obesity. This is an integrative literature review conducted according to PRISMA 2020 guidelines, with searches performed in PubMed, Scopus, BVS, and SciELO databases, including clinical trials published between 2020 and 2025. A total of 10 studies were included, evaluating outcomes such as body weight, body composition, glycemic profile, lipid profile, inflammatory markers, and insulin sensitivity. The findings showed that both strategies promote significant weight loss, with no consistent differences between them, indicating that energy deficit is the main determinant of this effect. However, some IF protocols demonstrated additional benefits, including greater reductions in visceral fat, triglycerides, and C-reactive protein in populations with metabolic syndrome and type 2 diabetes. On the other hand, no additional metabolic benefits of fasting were observed in the absence of energy restriction. In conclusion, IF is an effective and viable alternative to CCR, presenting similar metabolic outcomes, and the choice of strategy should be based on adherence and individual characteristics.
